What is Sciatic Nerve Endometriosis?

Sciatic nerve endometriosis is a rare form of deep infiltrative endometriosis caused by the implantation of endometrial tissue around or within the sciatic nerve. It is usually associated with advanced-stage disease.

How Does It Occur?

It develops through the spread of endometrial cells along pelvic nerve structures. Sacral plexus and sciatic nerve involvement may be observed.

Most Typical Symptoms

Pain radiating from the hip to the leg that increases during menstruation (catamenial sciatica) is the most characteristic symptom. The pain is usually unilateral.

Pain Characteristics

The pain radiates along the sciatic nerve: it can extend to the hip, posterior thigh, calf, and foot. It can become chronic over time.

Neurological Symptoms

In advanced cases, numbness, tingling, muscle weakness, and loss of reflexes may be observed.

Gait Disturbance

In patients with severe pain, limping and difficulty walking may develop.

Relationship with Menstruation

The increase in symptoms during menstruation is a critical diagnostic clue and differentiates it from other neurological diseases.

How is the Diagnosis Made?

MRI is the most valuable imaging method, especially for nerve involvement. Clinical suspicion is very important in diagnosis.

T2 hypointense nodules around the sciatic nerve, nerve thickening, and fibrosis in the surrounding tissue may be observed.

Treatment Approach

Treatment is mostly surgical. Nerve-sparing excision techniques are applied. Medical treatment can reduce symptoms but is usually not sufficient.
