General Information
HPV infection is common and often transient in women under 30. Therefore, the decision for colposcopy should be more selective.
When is Colposcopy Needed?
In this age group, some smear results directly require colposcopy.
Definite Colposcopy Indications
• HSIL (high-grade lesion)
• ASC-H
• Cases with suspected cancer
LSIL / CIN 1 Status
• Follow-up is generally preferred over direct colposcopy
• Colposcopy is performed in persistent (ongoing) cases
ASC-US Management
• Repeat smear is recommended in the initial stage
• Colposcopy may be considered in persistent ASC-US cases
HPV Positivity
• Routine HPV testing is generally not performed
• However, if high-risk HPV (especially 16/18) is detected, colposcopy may be performed
When is it Performed Earlier?
• Immunocompromised patients
• History of high-grade lesion
• Patients non-compliant with follow-up
Colposcopy in Pregnancy
• Can be performed safely
• However, biopsy is carefully evaluated
Clinical Reality
It is important to avoid unnecessary colposcopy in patients under 30. Most lesions regress spontaneously.
