vNOTES (Incisionless Surgery)
Vaginal surgery; refers to surgeries performed vaginally using laparoscopic instruments without making any incision in the abdominal wall. In our country, the first oophorectomy in the field of gynecology was performed by me in 2016 and has taken its place in the scientific literature.
It can be applied especially in patients who do not desire children and need a hysterectomy due to large fibroids causing irregular menstrual bleeding, frequent urination, constipation, pain during intercourse, and persistent groin pain. Additionally, in patients who wish to preserve their uterus, it can be easily applied, especially for fibroids located close to the outer part of the uterus.
How Is It Performed?
Whether the uterus is completely removed or preserved, the abdominal cavity is accessed through 2-3 cm incisions made at the deepest point of the vaginal area. With the help of specially developed vaginal protective platforms for this surgery, the organs inside the abdomen are made visible. With the help of a camera and laparoscopic instruments, fibroids or the uterine supporting ligaments or vessels are appropriately separated and removed from the abdomen. The vaginal tissue is then repaired, completing the operation.
Advantages
Reproductive organs are immediately adjacent to the vaginal area, making them easily accessible through this route. Whether it's the removal of fibroids vaginally or just the removal of fibroids, the procedure takes an average of 30-45 minutes. Due to the short operation time and the absence of an incision in the abdominal area, early discharge is possible. Average discharge after surgery occurs 6-18 hours later.
