Egg Count (Antral Follicle Count – AFC) What Is It? – Comprehensive Guide

What Is Egg Count?

Egg count is the process of counting the small (antral) follicles found in the ovaries. This assessment provides important information about ovarian reserve.

What Is AFC (Antral Follicle Count)?

Antral follicles are small egg sacs, 2-10 mm in size, visible by ultrasound. The number of these follicles is an indicator of ovarian reserve.

How Is It Done?

It is performed via transvaginal ultrasound between days 2-5 of the menstrual cycle. The follicles in both ovaries are counted to obtain the total value.

What Do AFC Values Mean?

Low AFC: Decreased reserve.
Normal AFC: Good reserve.
High AFC: May be associated with Polycystic Ovary Syndrome (PCOS).

Egg Count and Age

Egg count decreases with age. This decrease accelerates after the age of 30.

AFC and AMH Relationship

When AFC and AMH are evaluated together, the most accurate information about ovarian reserve is obtained.

AFC and Pregnancy

High AFC may mean a better response in IVF treatment. However, quality is not always dependent on quantity.

Endometriosis and AFC

Endometrioma (chocolate cyst) and surgical procedures can reduce AFC.

Who Should Have It Done?

Women planning pregnancy, suspected of infertility, at risk of early menopause, and planning IVF.

Conclusion

Egg count is one of the most important methods in evaluating ovarian reserve and, when interpreted correctly, is guiding in treatment planning.
