Smear Result ASC-H: What Should Be Done? – Patient Information Guide
What is ASC-H?
ASC-H (Atypical Squamous Cells – cannot exclude HSIL) is a smear result indicating that the possibility of a serious (high-grade) lesion in cervical cells cannot be ruled out.
Important Information
ASC-H is considered more serious than ASC-US, and there is a higher probability of underlying HSIL (CIN 2-3).
General Approach
• Watch-and-wait approach is not recommended
• Further evaluation must be performed
What Should Be Done?
• Colposcopy is performed
• Biopsy is taken if necessary
What is Colposcopy?
It is a method that allows detailed examination of the cervix under magnification. Samples can be taken from suspicious areas.
Management Based on Result
• Normal / low-grade finding: follow-up
• If CIN 2-3 is detected: treatment (LEEP/conization)
• Further investigation in suspicious cases
HPV Test
While the HPV result is important in ASC-H cases, colposcopy should be performed even if it is negative.
Who Should Be More Cautious?
• Patients over 30 years old
• HPV 16/18 positivity
• Those with a history of abnormal smear
Approach During Pregnancy
• Colposcopy can be performed
• Biopsy is usually carefully evaluated
• Treatment is mostly deferred until after delivery
Clinical Reality
ASC-H absolutely requires further evaluation due to the possibility of an underlying high-grade lesion.
Conclusion
An ASC-H result is considered serious and should be evaluated with colposcopy without delay.
