Importance of Imaging in the Diagnosis of Adenomyosis

Although clinical findings are important in the diagnosis of adenomyosis, a definitive diagnosis is often made with imaging methods. Transvaginal ultrasound and MRI are particularly the most valuable methods.

Transvaginal Ultrasound (TVUS)

It is the first-line imaging method. It provides high accuracy in experienced hands and is widely used due to its easy accessibility.

Ultrasound Findings

Heterogeneous structure in the myometrium, asymmetric wall thickening, small anechoic cysts, 'fan-shaped shadowing', and myometrial striations are typical findings.

Junctional Zone Changes

Irregularity and thickening of the border between the endometrium and myometrium is an important finding in ultrasound and MRI.

Color Doppler Findings

Diffuse and irregular vascularization may be observed in adenomyosis. This finding can help differentiate it from fibroids.

Why is MRI Important?

MRI is the method with the highest accuracy, especially in suspicious or complex cases. It also plays a critical role in surgical planning.

MRI Findings

A junctional zone thickness of >12 mm is one of the most important criteria. Additionally, T2 hypointense areas and small hyperintense foci may be observed within the myometrium.

Focal vs. Diffuse Adenomyosis

While localized lesions are seen in the focal form, the entire myometrium can be affected in the diffuse form. MRI can clearly demonstrate this distinction.

Differential Diagnosis with Fibroids

Adenomyosis typically has ill-defined borders and shows diffuse spread. Fibroids, on the other hand, are better circumscribed and encapsulated.

Imaging and Fertility

Imaging findings guide fertility planning by evaluating the extent of the disease and uterine structure.
